[Cjk] full Unicode support added

I've extended CJK's `UTF-8' environment to support full Unicode up to
U+10FFFF.  Subfonts for characters greater than U+FFFF expect four
trailing hexadecimal digits in the subfont name (see attached
Unicode.sfd).
